Messalina Jordan, DO
Messalina Jordan, DO, was born and raised in urban Jacksonville, Florida. After working as a nurse for 23 years, she attended medical school at Philadelphia College of Osteopathic Medicine in Suwannee, Georgia and completed residency with a surgical obstetrics fellowship at Huntsville Family Medicine Residency Program in Huntsville, Alabama.
Dr. Jordan is Board Certified in Family Medicine, a member of American Academy of Family Physicians, Alabama Academy of Family Physicians, Albertville and Boaz Chamber of Commerce, and Boaz Rotary. Dr. Jordan believes her calling is to provide healthcare to people living in rural America, which started in Kansas and continues in the South, where she will continue to practice preventative, prenatal, acute and chronic conditions, but without the middle man. She looks forward to spending more time with patients, offering medication and laboratory savings, and providing same or next day appointments for her patients.

Dr. Jordan enjoys spending time with husband, four children, two step-sons, three daughters-in-law, and seven grandchildren. She attends the First Baptist Church of Boaz.

Breck Hamilton, AGPCNP-BC
Hello! I'm Breck Hamilton, hailing from the vibrant town of Boaz, AL! I proudly graduated from UAB in 2022 with a Master's in Nursing, specializing in adult gerontology. Before earning my master's, I had the incredible opportunity to gain hands-on experience at UAB and embark on exciting travel assignments in Tuscaloosa and Tennessee. I can't wait to bring my skills and knowledge back home to make a positive impact in the Boaz community and beyond!

Katie Beth Casey, MSN, FNP-C
Hi! I’m Katie Beth Casey, and I was born and raised in Boaz, Alabama. I proudly earned my Master of Science in Nursing with a Family Nurse Practitioner concentration from the University of North Alabama in 2024. This advanced training allows me to care for patients of all ages and stages of life.
Before obtaining my master’s degree, I had the privilege of serving patients at Children’s of Alabama for four years and at Marshall Medical Center North for two years. These experiences strengthened my passion for providing compassionate, high-quality care.
I am thrilled to be part of the Boaz and Albertville Family Care team, where I can continue serving my hometown community and helping families receive the quality healthcare they deserve—right here at home.

MEMBERSHIP FEES
Boaz & Albertville Family Care has established a straightforward age-based membership fee structure. Please note, patients under 20 must be enrolled concurrently with a parent or guardian, otherwise the fee is $35 per month. Please schedule a free, no-obligation 30 minute meet-and-greet with Dr. Jordan to see if our practice is right for you!

Labs and procedures will be billed at the time of service. A $99 registration fee (one registration fee per family living in same household) is required. Members paying annually will receive a 10% discount. Companies enrolled with five or more employees will have registration waived and 10 or more employees will have registration waived and receive a 10% discount. We reserve the right to dismiss members for missed payments. Back payments of membership, plus 3-6 monthly and double registration fee will be required for re-enrollment.
No chronic pain or benzodiazepines management.
